With the winding edge selected now you want to extrude that edge out using the extrude tool while in edge mode AND holding down the Ctrl key (windows) or Command key (mac). That will allow the edges to extrude out all at the same time.
Now that the threads have been extruded from the cylinder you will want to scale the whole shape down. Your shape should look something like the image below once you have finished scaling it down.
Now you will need to rotate the view so you can see the bottom of the now emerging screw. While in face mode select the end face of the screw.
Once the end face is selected you will need to extrude the face out like the image below. Holding down the Ctrl key (windows) Command key (Mac) will keep the extrusion on the face normal. Sort of like holding shift down in Illustrator.
and again so there is two sections to the extrusion.
Select the faces around the top of the screw using shift select to select all of the faces. Then, using the extrude tool, extrude all the faces at once to make the head of the screw. Rember to hold down the Ctrl (windows) or Command (mac) keys down to extrude all the faces at the same time.
On the other end select the end face and extrude it out like the image below.
With the end face selected you need to scale it down using the scale tool. The image below shows how it will look. Hold down the Ctrl (windows) Command (Mac) key while you scale it down.
Back to the head. Select the bottom edge of the head and use the loop select option to select the edges all the way around the bottom edge. Once the edge is selected all the way around use the bevel tool to make a small bevel. This will make a realistic edge to the bottom of the screw head.
To make the slot at the top of the head first you need to connect the points at the top of the screw head with the connect tool. See the image below .
Once the connection has been made select all the edges that make the edge that crosses the top of the screw head. With the edge selected use the bevel tool to make the beginnings of the slot.
Now, select the faces that make up the slot and using the extrude tool push the faces in to form the slot. Holding the Ctrl (windows) Command (Mac) key will constrain the extrusion down.
Select one of the spoke edges on the top of the screw head and right click or control-click, then chose ring select.
Now connect all the spoke edges together using the connect command or using the "C" key.
